How will a user "integrate" an external survey tool?

Under document > settings there's a placeholder menu item called "Integrations & Services". I could also move this to the workspace level if that made sense (it would depend on the integration API). Someone would register with the survey app they want to integrate with, follow some basic instructions on how to obtain their API key, then insert their API key under the (for example) SurveyMonkey integration screen in our app. There could also be some integration settings to configure. From there, they could follow a wizard of some kind to link a document with an existing survey. Similar instructions could be followed to integrate with Slack and Basecamp and so on.

The "Integrations & Services" nav item will be removed before launch since we have no integrations yet. For integrations like Slack, it would make sense to integrate at the workspace level instead of the document level.

After discussions, we agree that it was within scope to deliver a Poll feature. This will work like a Twitter poll within the Issue feature. This replaces most of the need for feedback in the format of rigid question and answer. More elaborate survey features will be an integration, most likely with Survey Monkey.

The “Formfacade script URL” is a perfect example of a survey integration which we've had since June 16th 2020. Works well.
